Nursing care

  1. Assessment:

    • Conduct comprehensive health assessments, including physical, psychological, social, and environmental factors.
    • Collect vital signs (temperature, pulse, blood pressure, respiratory rate).
    • Perform a detailed patient history, including past medical history, allergies, and current medications.
    • Assess pain using standardized pain scales.
    • Evaluate the patient's level of consciousness using tools like the Glasgow Coma Scale (GCS).
  2. Diagnosis:

    • Analyze assessment data to identify nursing diagnoses.
    • Formulate nursing diagnoses using standardized nursing classification systems (e.g., NANDA-I).
    • Collaborate with other healthcare professionals to establish medical diagnoses.
  3. Planning:

    • Establish measurable and achievable goals in collaboration with the patient and their family.
    • Develop a comprehensive care plan that addresses the identified nursing diagnoses.
    • Prioritize nursing interventions based on the patient's needs and acuity level.
    • Set timelines for achieving goals and reassess periodically.
  4. Implementation:

    • Administer medications and treatments as prescribed by healthcare providers.
    • Provide patient education on disease processes, medications, and self-care techniques.
    • Assist with activities of daily living (ADLs) such as bathing, dressing, and grooming.
    • Perform wound care and monitor for signs of infection.
    • Implement fall prevention strategies, including keeping the environment free of hazards.
    • Encourage mobility and perform range of motion exercises as appropriate.
    • Administer intravenous (IV) therapy and monitor IV sites for complications.
    • Monitor intake and output (I&O) and maintain fluid balance.
    • Provide emotional support and counselling to patients and their families.
    • Collaborate with interdisciplinary team members to coordinate care.
    • Document all interventions and patient responses accurately and timely.
  5. Evaluation:

    • Assess the effectiveness of nursing interventions in achieving desired outcomes.
    • Compare actual patient outcomes to expected outcomes.
    • Modify the care plan as needed based on the patient's response to treatment.
    • Provide feedback to the healthcare team for continuous improvement in patient care.
    • Educate patients and families on strategies for managing ongoing health needs.
